A Diabetes MedsCheck involves a one-on-one consultation with your Pharmacist. The Pharmacist will review your type 2 diabetes management plan in store to help you:
A Diabetes MedsCheck involves a one-on-one consultation with your Pharmacist. The Pharmacist will review your type 2 diabetes management plan in store to help you:
- Improve your understanding of your diabetes medication;
- Use your blood glucose monitoring devices more effectively;
- Manage your blood glucose levels;
- Understand the best time to test your blood glucose;
- Reduce the risk of developing complications associated with type 2 diabetes.
Who is eligible for a Diabetes MedsCheck?
- Be a Medicare and/or DVA cardholder;
- Have not received a MedsCheck, Diabetes MedsCheck, Home Medicines Review (HMR) or Residential Medication Management Review (RMMR) in the last 24 months;
- Be living at home in a community setting;
- Have recently been diagnosed with type 2 diabetes (in the last 12 months) or have less than ideally controlled type 2 diabetes; and
- Be unable to gain timely access to existing diabetes education /health services in their community.
- You do not require a referral from your GP.
How much does the service cost?
The service is FREE to eligible patients and fully subsidised by the government.
